W2 and 1095C Notice

The Payroll Office is working to reconcile and distribute your 2016 W-2’s AND 1095-C’s as efficiently and timely as possible.  The 2016 W-2’s AND 1095-C’s will be mailed by Tuesday, January 31, 2017, as required by the Internal Revenue Service for those employees who do not consent electronically. 

We encourage you to select the option of receiving an electronic W-2 AND 1095-C.  In order to receive your W-2 AND 1095-C via banner web, you must submit an electronic consent form.  The deadline to submit the consent form is Friday, January 13, 2017. 

You can submit your W-2 AND 1095-C consent forms by following these steps:

  1. Log in to Banner Web
  2. Click on Employee Information
  3. Click Tax Forms
  4. Click Electronic Regulatory Consent
  5. Check the “Consent to receive W-2 electronically” Box
  6. Check the “Consent to receive 1095-C electronically” Box
  7. Click Submit

** To verify that your consent has been successfully submitted a GREEN check mark with the following statement will appear at the top of the page after you have clicked submit. “Electronic Statement consent was submitted successfully”. **

However, if you do not submit an electronic consent form by January 13, 2017, your W-2’s AND 1095-C’s will be mailed via U.S. Postal Service.
