Annual Assessment Cycle Administrative and Academic Support Units Reporting Forms
As we continue to refine our system for documenting institutional effectiveness, some changes in the reporting system for our annual assessment efforts. We will continue to plan our assessment in the fall and report assessment results and how they were used in the summer, but the forms for reporting have been changed. There are two sets of reporting forms: one for Instructional Programs and one for Administrative and Academic Support Units.
Annual Assessment Cycle Administrative and Academic Support Units
Please download the administrative and Academic Support unit forms and submit completed Forms A and B to the Office of Institutional Effectiveness and Planning.

Instructional Programs
Please download these forms. Please submit Form A and B to the Office of Institutional Effectiveness and Planning by September 15, 2002. Form C should be completed by July 15, 2003 and submitted to the Office of Institutional Effectiveness and Planning. Copies should also be maintained in the department office.
